Hello Lindop Families,

The Loyola Mobile Clinic will be back at Lindop on Thursday, 6/24/2021, from 10AM-2PM. The mobile clinic is able to provide physicals, sports physicals and necessary immunizations by appointment only. Masks will be required by all while on the mobile unit.

**PLEASE BE ADVISED IF YOUR CHILD IS COVERED UNDER PRIVATE HEALTH INSURANCE, THEY ARE UNABLE TO BE SEEN BY THE MOBILE CLINIC. WE APOLOGIZE FOR THE INCONVENIENCE**

The following grades require updated physicals per Illinois state law, due by October 15, 2021:

--PreK (first year)
--Kindergarten
--6th Grade

While return to sports has not officially been announced, 5th through 8th grades will be required to have a sports physical on file in order to try out. If your child is eligible to be seen by the mobile clinic, please consider having a sports physical completed.

School health requirement waivers:

Any request to waive immunizations or physical examination based on religious beliefs must be submitted on the IL Certificate of Religious Objection Form

If you have questions regarding immunizations or other school health requirements, please contact the health office.

Medications at School:

  • If your student requires prescription or over-the-counter medication to be administered during school hours, a completed Medication Authorization Form (currently under revision) signed by a licensed health-care provider and parent/guardian must be on file in the health office. These forms must be renewed annually at the beginning of each school year. You may obtain a current form from the health office.
  • All medication must be delivered to the health office by a parent/guardian.
  • All medications must be stored in the health office, with the exception of students who are approved to self carry an asthma rescue inhaler or epi-pen.

Emergency Action Plans:

Food Allergies

  • If your student will require a substitution to the typical school lunch due to an allergy, a Physician Statement for Food Substitution must be on file in the health office. This form must be renewed annually at the beginning of each school year.

Student Illness:

  • In general, students who become ill at school with the following symptoms will be sent home for rest/recovery:
    • Temperature greater than 100 degrees F by mouth.
    • Vomiting.
    • Diarrhea.
  • Severe cold/flu symptoms that prevent the student from participating in classroom activities or that the student is unable to manage using basic cough/cold/hand hygiene.
  • Students must be symptom and fever free for 24 hours (without the use of fever reducing medication) prior to returning to school.
  • If you have questions regarding your student's return to school after an illness, please contact the health office.

Care in the Health Office:

  • Basic first aid, comfort measures and prescribed medication administration are routinely provided by the registered nurse in the health office.
  • The student's parent/guardian will be contacted for any visit to the health office that involves a head injury or requires care beyond basic first aid.
  • If you would like to be contacted each time your student visits the health office, please notify the nurse in writing.
